Authorization of Autopsy Examination

The regulations in effect are the result of the requirements of the "Human Tissue (Scotland) Act" 2006. Authorisation must be obtained using the appropriate national forms which are available with information booklets for relatives

Authorisation
  • A relative of the deceased should not be invited to sign this form if the hospital staff are aware of objections expressed by the deceased.
  • The nearest relative as defined in the information booklet, or the patient’s nominated representative, must give authorisation for the examination. Authorisation must be obtained ‘face to face’. Obtaining authorisation over the phone is not permitted. Authorisation cannot be given by hospital staff.
  • Any limitation on the extent of the examination or the removal of tissue expressed by the person giving consent should be strictly observed.
Witness of Signature
  • The signature of the individual consenting to the autopsy should be witnessed by someone other than the individual who has explained the procedure.
