Supplies of request forms, specimen containers and specimen bags can be obtained from  Astley Ainslie Distribution Centre. Telephone 0131 537 9277 (ext. 49277) for further details.

Please do not have multi disciplines on 1 request form.

The following requests forms (for use where electronic requesting is not available or practical or for use in downtime of the electronic systems) are in use:
