For patients on high dose methotrexate, the dose and duration of folinic acid rescue are determined by plasma methotrexate concentrations, measured at 48h after the start of methotrexate administration, again at 72h and then every 24h until plasma methotrexate is <0.1 µmol/L, when rescue is deemed to be complete.
